The University of Arkansas vision insurance plan is a fully insured group plan through Superior Vision. All claims are processed by Superior Vision.
Vision insurance helps lower your out-of-pocket costs when you purchase frames, lenses or contacts, or when you get a routine eye examination. While the vision plan is open to all benefit-eligible employees, it is really intended for those who wear glasses or contacts. It does not cover medical problems associated with your eyes. Since you will likely only use the benefit once a year (the plan year is the calendar year), be sure to go to a provider in Superior Vision’s network to get your best benefit.
You may choose between two plans, Basic or Enhanced. The difference in benefits is the allowances for frames and lenses.
The premium cost depends on which of the two plans you elect and the family members you are covering.
The vision plan requires plan year participation. Once you’re enrolled, you will not be able to cancel coverage or delete eligible dependents until the end of the calendar year.
You can cover your spouse, as legally recognized by the State of Arkansas. You can also cover your children until they reach their 26th birthday, regardless of their marital, employment or student status.
New employees may enroll within the first 31 days of date of hire by completing the required enrollment forms. Benefit coverage will begin on the first day of the month following the date enrollment forms are received in the Human Resources Office.
If you are already enrolled in the vision plan and gain a new family member by marriage or birth/adoption, you have 31 days to add that person to your coverage.

Resignation - Under the federal regulation called COBRA (Consolidated Omnibus Budget Reconciliation Act), you have the option of continuing your current group vision insurance coverage for up to 18 months after termination (or 36 months under certain conditions). To keep this coverage, you must pay the total premium (employee and employer share of the cost) plus 2%.
Retirement – Vision Insurance may not be continued into retirement.
