University of Arkansas Group Benefits

During your first 31 days of employment, you may choose to enroll in any of the University of Arkansas group benefit plans. Every employee must complete and return the University of Arkansas Group Benefits Enrollment Form - EBEN-227 (PDF).
To enroll you or your eligible dependents in any of the benefits in the University of Arkansas Group Benefits Program, you must complete the University of Arkansas Group Benefits Enrollment Form - EBEN-227 (PDF) along with any enrollment forms required for the optional benefits. Even if you do not want any of the optional coverages, you must complete the University of Arkansas Group Benefits Enrollment Form in order to indicate your beneficiary for the free Basic Life Insurance. You have thirty-one days from your date of hire to enroll by returning completed enrollment forms to the Extension Human Resources Office. If you gain new dependents through marriage or birth, you may enroll them within thirty-one days of the event without showing proof of insurability.
The automatic free coverages (Basic Life Insurance and Basic Long Term Disability) are effective from the date of hire. However, all other insurance coverage becomes effective the first day of the month following the date that the completed enrollment form is received in the Human Resources Office.
Dependents - Eligible dependents are your spouse and children under age 26. They can be married and do not have to be living with you.
