General Information
During your first 31 days of employment, you may choose to enroll in any of

New hires wishing to participate use the following forms:
Every employee must complete and return the
University of Arkansas Group Benefits Enrollment Form
- EBEN-227 (PDF).
Enrollment - To enroll you or your dependents in any of the benefits
in the University of Arkansas Group Benefits Program, you must complete the
University of Arkansas Group Benefits Enrollment Form
- EBEN-227 (PDF) along with any
enrollment forms required for the optional benefits. Even if you do not want any
of the optional coverages, you must complete the University of Arkansas Group
Benefits Enrollment Form in order to indicate your beneficiary for the free
Basic Life Insurance. You have thirty-one days from your date of hire to
enroll by returning completed enrollment forms to the Extension Human Resources
Office. However, you are encouraged to return the forms as soon as possible
because most of your benefit coverages will begin on the first day of the month
following the date your paperwork is received in the Human Resources Office. If
you gain new dependents through marriage or birth, you may enroll them within thirty-one days of the event without showing proof of
insurability.
Both automatic free coverage (Basic Life Insurance and Basic Long Term
Disability) are effective from the date of hire. All other insurance coverage
becomes effective the first day of the month following the date that the
enrollment form is received in the Human Resources Office.
Dependents - Eligible dependents are your spouse and your unmarried
children under age 19. Unmarried children ages 19 through 24 are eligible if
they are full-time students and are designated as your IRS dependents.
